Insurance & Payment Information
Over the past few years, healthcare reform has had a positive impact on many preventive wellness services, and as a result, your health insurance may cover the entire cost for your nutrition counseling.
Nourished Lifestyles participates with the following insurance companies:

The number of visits, co-pays, and deductibles vary from plan to plan. Even though we may be in network with your insurance provider, it does not guarantee coverage. We will work with you to contact your insurance company to determine your coverage.
Please note that insurance policies vary greatly and coverage is from contract year to contract year. So we will ask to verify your coverage year to year.
Also note, a physician referral does not guarantee coverage.

Medicare
At this time we do not participate with Medicare, therefore we are unable to bill your supplemental Medicare policies.
However, if your secondary insurance is independent to Medicare, then coverage may be possible.
Additionally, many Medicare Advantage plans have wellness money available to you. This could be used as reimbursement for your nutrition counseling. You would pay for the services up front and then submit your receipt to your plan, and they would reimburse you to the maximum amount allowed.
Cancellation Policy
Cancellations and re-scheduled sessions may be subject to a full charge if not received at least 24 hours in advance. This is necessary because a time commitment is made to you and is held exclusively for you. If you are late for a session, you may lose some of that session time. A canceled or missed session cannot be billed to your insurance company. You will be responsible for paying this cost out of pocket.
